Narrative

The Adult Foster Care (AFC) program allows people to receive personalized care from a family member or friend while remaining at home. This program provides financial support to caregivers. This lets them to focus on the well-being of their loved one. AFC is ideal for those who require daily assistance but prefer to stay in a familiar home setting. Member Eligibility:

• Must be 16 years or older

• Require assistance with at least 1 ADL (Activity of Daily Living)

• Must have MassHealth Standard

• Must have a family member (can’t be spouse or parent, if the member is a minor or guardian) who lives in the same home.
