Narrative

Shared Living is a residential choice for people with disabilities (PwD). They have can live in private family homes with dedicated caregivers. Each person plays an active role in planning their daily routine. They get services tailored to their needs through LifeStream’s clinical team and experienced professionals.

Adult Family Care is a program that gives a residential option to people who cannot safely live alone. But, they do not need the level of support of a more traditional group residence. AFC adults may move into the homes of host families. They may stay at home with their family members. Family members help with daily care and the normal supports of everyday. They live in the privacy and comfort of a home space.
