In Home
Assistance with details of daily living, such as home care, nutrition and cooking skills, money management and safety
Community
Assistance with spending time in the community, such as attending class, visiting museums or parks, and spending time with friends
In-Home Support Models
Traditional Model
A provider agency has responsibility to plan programs for the person you support.
Self-Directed Models
Self-Direction occurs through either an Agency with Choice (AWC), a provider that works with you to manage the budget, or a Participant Directed Program (PDP). This is a program in which the individual or family has the responsibility for hiring staff, managing the budget and paperwork.
