Narrative

To be part of this program one must 16 years or older,

be eligible for MassHealth coverage and need help  to complete certain activities of daily living (ADLs). ADLs are bathing, eating, dressing, walking or toileting.

A team consisting of a Registered Nurse and a Care Manager conduct the initial screening. After acceptance into the program, they will make ongoing home visits to provide:

• nursing
• case management and
• clinical supports

In addition to these supports there is also a financial pay given to the caregiver. Pay is based on the level of helpthat the member needs.
