Narrative
Bay State AFC LLC provides nursing services by a Registered Nurse and care management by a Social Worker with a periodic home visit to train, provide support, health education topics, and monitor clients’ health status and adjustment to the Adult Foster Care program. The care giver will receive training, support and supervision to assure the client’s well being.
