Are you looking for a family home setting for care? AFC is designed to provide supportive family living to elderly or disabled people who can no longer live alone. They would like a family setting over an institution. AFC provides assistance with activities of daily living (ADLs), nursing oversight, and care management at home with 24 hour supervision. Both participants and caregivers must meet criteria and guidelines set forth by MassHealth. 

Group Adult Foster Care (GAFC) is designed to support and assist those who are elderly or disabled but able to live on their own. Services are designed to increase people's overall health and safety by providing assistance with activities of daily living (ADLs), supervision and encouragement with an individual’s personal care needs.
