Narrative

We provide support and supervision that ensure safety. We work on fostering deep personal connections that promote well-being and successful outcomes for our people. This service offers the person the tools needed to maintain a living arrangement, either in the family home or living independently.

Supports are given for less than 24 hours per day. They are not available for people living in a residential facility or group home. People must be referred to Empower by their DDS Service Coordinator. People who have services through DDS and are interested in learning more about Empower should call us for more information. Services range from as little as 1-2 hours a week or as many hours as are needed. Services are approved by the funder to support the person.

The DDS Supervisor may act as a PCA surrogate for those people in Individualized Home Supports. They are also using a MassHealth-funded PCA program.
