Narrative

The mission of our DDS funded program is to provide innovative and individual support. We lessen barriers and empower people to live the lives they choose.

The Home & Community Connections (H&CC) Division takes pride in our rich history of consistent, high-quality supports. Supports are built around each person's needs. We are committed to self–determination. We always promote options for the people we support to have control of their lives and the supports that they receive.

Our program practices are designed to provide supports in a dignified way. We honor the person's wants. We empower the people supported to control their services to the greatest degree possible. Each person is unique.  Effective support strategies and services must be adapted to. They are built around the specific personality and need of each of them.

Our approach to designing and implementing supports is person-centered. All supports are provided using trauma sensitive practices. We believe that healing happens in relationships. H&CC practices are continually examined and improved while questioning our own assumptions.
