Narrative
In our Adult Foster Care (AFC) program, adults with disabilities receive care in a family setting. Funded by MassHealth, our focus is on supporting people in a familiar home setting. We provide:
- care coordination
- medical oversight
- support from professionals
- including nurses and case managers
Regular home visits ensure services are provided seamlessly. Our goal is to maintain people in a family home. We offer needed training, respite care, and access to community resources for good support.
In our Shared Living (Placement Services) program, adults with disabilities and those with brain injuries find care in a family setting. Waystone provides complete support and home visits including:
- nursing
- case management
- training
- respite care for people in our programs
- financial payment
